POSITION: Infant Case Manager

DESCRIPTION:
The Infant Case Manager is responsible for carrying out prescribed activities within designated target areas.  This position will work with a private pediatrician, the State Bureau of Child welfare and the Early Head Start grantee in Milwaukee (Next Door Foundation) to identify infants in need of care coordination.  This position will also participate in the on going care as well as identification and enrollment of infants into the Milwaukee Healthy Beginnings Project (MHBP).  This individual will connect these infants to linkages with medical homes and other social systems in the community to ensure good infant health.

 REPORTING: This position reports to the Project Coordinator

 Duties and Responsibilities:

  1. Recruit, assess and enroll infants in the MHBP case management program from targeted areas.

  2. Conduct aggressive outreach activities and work with a private pediatrician and the Early Head Start grantee in Milwaukee to identify infants in need of care coordination.

  3. Target area residents within specified zip codes enrolling post partum women or women in the interconceptional period and their infants in the MHBP if not currently enrolled, providing full and complete documentation per project protocols.

  4. Will identify pediatric provider and/or a medical home on the enrollment form.

  5. Provide parents of infants with information and education on infant health care.

  6. Provide regular client contact through appointment, phone or home visits, identify client care problems and collaborate with providers, WIC, nutritionist, mental health and AODA facility for referrals and to ensure the delivery of early, consistent and appropriate infant health care, including immunizations.

  7. Assist in the assembling and submission of data collected.

  8. Must attend internal project meetings as well as project training and other project events and committee meetings as directed.

  9. Work with internal and external staff and/or providers to establish and maintain coordination of client services.

  10. Develop individual care plans as needed.

  11. Other duties as assigned.

Qualifications:
Graduate of an accredited LPN nursing program, licensed in the State of Wisconsin or have a degree in Social Welfare, with a Bachelors or Masters preferred, (a two year degree with related experience will be considered).  One to three years related experience in pediatric and/or perinatal health care a plus.

Experience in public health or community nursing and bilingual in Spanish preferred, but not required.  Individual must be representative of the target population to be served.  Ability to operate independently and in a team context. Have good oral and written communication skills. Must reside within the city of Milwaukee, having a stake and familiarity with the project area. Have considerable flexibility with working hours (some evening and weekend hours will apply).  Must have reasonable problem solving skills.  Must have a valid driver’s license and auto insurance. 

Is a salaried position, commensurate on experience and qualifications.
